Please complete and submit this form. I base what I pay for your items based on three simple questions that are admittedly open to interpretation. Plus photos and any information you provide. This is a simple business transaction. No more, no less.

1. OVERALL APPEARANCE. The foot bed must have a clear imprint of heel, arch, ball, and toes. The darkness, clarity, and thickness of your foot imprint is the most important factor. The general condition of the item is not important.

2. ODOR. Most people can’t or find it difficult to identify their own scent. Be as objective as possible. HINT: Spray some clean water onto the foot bed until it is slightly damp. This usually amplifies the odor and makes it easy to identify. Wait until the footwear dries before placing it into a plastic bag or it will become musty & moldy. Wait until it's dry.

3. ACCESS TO THE FOOT BED. Accessibility to the foot bed is somewhat important but by no means a deal breaker.
